Description: Assumption of Risk, Waiver and Release Form

The Assumption of Risk, Waiver and Release Form will be completed by all students attending travel via commercial air transport sponsored by student organizations for educational and cultural enrichment.

This form will be given to the Security Department and a copy will be filed in the Student Activities Office.
