Description: Jury Duty


Employee:

Informs supervisor of notification of jury duty service.

Determines as precisely as possible period of expected absence.

Forwards copy of jury duty summons to Human Resources Department.

Prepares and submits Leave Request Form indicating dates(s) of absence to supervisor.

Endorses and forwards to Business Office court-issued check received for services as a juror.

Retains court-issued funds received for travel and meal allowances.

Supervisor:

Approves release time and makes arrangements for job coverage.

Receives, reviews, and approves Leave Request Form and forwards white copy to Human Resources Department.

Human Resources Department

Forwards jury duty policy and procedure to employee upon receipt of jury duty summons.

Enters absence data as reported on Leave Request Form in Human Resources attendance and leave system.
