Blackboard Course Shell Request Form

This form is intended for faculty to request a Blackboard course shell for online, web enhanced, and hybrid courses. All Blackboard course shells must be requested using this form. If you do not complete this form you will not have course shells created for you. All Blackboard course shell requests must be submitted minimally one week prior to the start of the semester or you will not have a course shell created for you for that semester.
